SASH® (Support and Services at Home) is a free, statewide wellness program that helps older adults and individuals with disabilities maintain independence, improve health, and enhance quality of life.
Through SASH, participants receive:
- Personalized support from a dedicated SASH® coordinator and wellness nurse
- Health and wellness coaching tailored to individual needs
- Group programs and activities promoting social connection and well-being
- Assistance navigating healthcare services and community resources
Who Can Participate?
SASH® is available to older adults and individuals with disabilities living at Cornerstone Housing Partners Senior properties and in select surrounding communities. Participants must be receiving Medicare to receive SASH services.

SASH® empowers participants to take charge of their health, prevent hospitalizations, and remain safely in their homes. By fostering stronger community connections and access to essential services, SASH® helps participants age in place with confidence.
How It Works
- Participants are assigned a SASH® Coordinator and a Wellness Nurse, who serve as trusted guides and advocates.
- The SASH® team coordinates care with healthcare providers, social services, and community organizations.
- A variety of health and wellness programs—from fall prevention to nutrition workshops—are available at no cost.
What About My Privacy?
SASH® staff follow the same privacy laws (“HIPAA”) as your doctor. We take your privacy and the control over your information very seriously. Your coordinator will go over the HIPAA regulations with you so you know exactly how SASH® uses and guards your information. SASH® is an “evidence-based program,” which means that staff report data on health indicators such as blood pressure readings. This way SASH® can analyze the data to document how the program is working.
